Include the following details:
1. your age as of December 31st, 2013
2. you personal mso registration number - if you haven't requested it yet, provide Duane you date of birth and address

3. list of the events you'd like to swim (click here to view list)
4. include a race time for each event you'd like to swim (even if you have to guess)
5. add that you're one of coach Mike's St-Laurent Sharks swimmers, but want to be registered with Technosport for this event
6. Duane will register you for relays, please indicate if you don't want to swim certain strokes (ie, I can't swim breaststoke because I can't do the proper breaststroke kick, bad knees!)
Duane will then forward you confirmation of your registration in the swim meet, along with payment details.
